Support Coordination
Support Coordination is an NDIS funded support that is designed to assist participants to make the most out of their NDIS funds. A Support Coordinator is a key person who can help make your NDIS plan come alive.
They work closely with you and other service providers to balance your funding arrangements, support needs and help you achieve your goals. At Community Support we work with you to ensure you are supported in all aspects of your NDIS Plan and empower clients to make choices around their wellbeing, service delivery and lifestyle.
Community Support is an NDIS approved provider to offer Support Coordination and Case Management (for Continuity of Support/ Fee for Service clients) in the categories below.
Support Connection
We provide some assistance in starting a client’s plan, connect clients with other providers and monitor plan progress. We also provide Assist Life Stages and Transitions services to prepare clients for the challenges they may face when undergoing life stage transitions and help them to overcome any barriers.
Coordination of Support
We provide assistance in starting a client’s NDIS plan, monitor and address any issues or barriers that may arise regarding the delivery of supports in accordance with the plan.
Specialist Support Coordination
We provide the same supports as level 2, but with the requirement for a specialised framework necessitated by specific high-level risks/needs.
Capacity Building and Training in plan and Financial Management
We provide assistance with capacity building and training in plan administration and management with a client, to strengthen their ability to undertake tasks associated with the management of their supports.
Recovery Coaches
Recovery Coaches will be available through your NDIS plan under Support Coordination. Recovery Coaches will provide support to people with psychosocial disabilities and help them to live and take control of their lives and manage their day to day living needs. Our skilled coaches will work collaboratively with you, your carers and families to help implement a daily living plan to help you live your life your way. They will assist you with your NDIS supports and link you to other providers as required to help you manage your everyday needs.
Roles and responsibilities of recovery coaches include:
o Developing a recovery-enabling relationship
o Supporting participants’ engagement with the NDIS
o Coaching to increase recovery skills and personal capacity, including motivation, strengths, resilience and decision-making
o Supporting participants with their recovery planning
o Collaborating with the broader system of supports.
